Facilities and Stakeholders
Acentra Health, formerly Kepro, is honored to have been selected as Nebraska’s new PASRR vendor! Since January 1, 2021, PASRR Level I screenings have been successfully completed through Acentra's online portal with instant results! Acentra Health also completes Resident Reviews for significant change and PASRR Level II evaluations and determinations via the online portal.
Our team utilizes a multi-disciplinary approach and consists of PASRR professionals such as, psychologists, nurses, social workers, and psychiatrists who work together to evaluate the appropriateness of nursing facility placement and the need for Specialized Services for each individual referred for a PASRR Level II evaluation and determination.
Acentra adheres to the Federal and State regulations governing the PASRR process. The Level II evaluations are sensitive to each individual’s culture, language, ethnic origin, and means of communication. Our determinations are person-centered and support individuals in need of additional services while in nursing homes.
Information on PASRR
PASRR was created as part of the Omnibus Budget Reconciliation Act of 1987 (OBRA 1987). The PASRR requirements, added to the statute as sections 1919(b)(3)(F) and 1919(e)(7) of the Social Security Act required that states create a process to assess the needs of individuals with Serious Mental Illness (SMI), Intellectual and Developmental Disability (ID), and/or Related Conditions (RC) entering into or residing in a Medicaid Certified Nursing Facility to ensure appropriate placement and services in nursing facilities.
Individuals with SMI, ID, and/or RC have special protections under state and federal law to ensure that long-term services and supports are individualized and provided in the right setting to meet their needs. Kepro’s PASRR program provides protections that align with Federal and State laws under the Americans with Disabilities Act (ADA) to ensure that anyone with SMI, ID, and/or RC is served in the least restrictive and most appropriate setting.
Acentra Health contracts with the Nebraska Department of Health and Human Services (DHHS) and Division of Behavioral Health (DBH) to serve as the Nebraska PASRR Level I and Level II vendor. We partner with hospitals and nursing homes throughout the state of Nebraska to provide PASRR Level I screenings and Level II evaluations and determinations. Level I screenings are completed in our system by hospital and nursing home providers. When a Level I is positive, Acentra's highly skilled team of professionals will conduct face-to-face evaluations throughout the state of Nebraska.
